New Patients

TBCHS is accepting new patients at all of our health centers. We particpate with Medicare & Medicaid and most insurance plans. Discount plans are available for most services based on family size and income. As a Patient Centered Medical Home, we have a team of providers and care staff who will assist you with your healthcare needs. You have the option to choose your assigned Primary Care Provider (PCP), however, please note that in certain circumstances we may need to assign you to a specific provider. Most of your appointments should be with your assigned PCP, however you may need to see another provider if your PCP is not available.

Scheduling an Appointment

Please contact any one of our health centers to schedule an appointment. Contact information can be found on our Contact Us page. Or just stop in! We have same-day appointment access and a walk-in clinic at each of our locations in the afternoon, every day. We also offer extended hours for anyone who is unavailable from 8AM to 5PM. Please contact our health centers to get more information on our extended hours availability.

What to Bring to Your Appointment

Someone from our office will contact you a few days prior to your appointment to review your reason for visit and to ask you a few questions so that we are more prepared to meet your needs when you arrive. Here is a list of some things you can plan for before you come see us:

  • Arriving early (15 minutes) prior to your scheduled appointment helps to complete paperwork and any pre-visit processing. Please make sure to bring your photo ID, insurance information and proof of income (tax forms, pay stubs, bank statements, etc...)
  • If you need transportation assistance to make it to your appointment, please contact the health center a few days in advance of your appointment and they will assist you in arranging transportation.
  • If you have seen another provider since your last visit, it would be helpful for us to receive a copy of your records so that we may have a better understanding of any changes in your healthcare needs.
  • Please come prepared with a list of the medications you are currently taking. Mark which medications you would like to request a refill for.
  • If you have any paperwork or forms that need to be completed by your provider, please bring the forms with you.
  • Make sure to have a list of any questions you may have with your provider. We want to make sure we are fully meeting your expectations for your visit with us.
